Client Goals Leave Andover Subacute Rehab Center to go home with family Will practice toilet routine/potty training Will eat healthier Free be of any behavioral incidents Will take all her medications daily Maintain healthy relationship with family Will call her family at least twice a week Will talk to therapist three times a week to discuss issues about family Will express her feelings in a journal daily Will express issues with family without cursing or yelling To Be Independent Will go to the bathroom herself once a day Will take her own showers at 5 times a week within the month Will brush her teeth without help Attend resident council meetings Needs structure Take care of personal hygiene Will make a planner Exercised at four days out of the week Will meditate and pray when she wakes up These goals were selected by the client, the client’s family, and the facility. Leah is well aware that she needs help in order to accomplish the goals she has set. With those goals selected, the client and I were able to come up with ways that would benefit her a great deal. Leah’s concern was that she would not be able to accomplish her goals. I told her that she has a great support system who is here to help her, and we will do whatever it takes to help her accomplish her goals.
